5）治疗同盟难以达成一致，往往协商治疗同盟，就需要50次左右。来访者会攻击治疗同盟的关系配对——治疗师扮演一个有关怀、有能力的医生角色，来访者扮演一个合作的、执行自我疗愈任务的好病人。尤其需要注意的是，不少个案本身是心理咨询师，他们攻击治疗同盟的方式是“老师移情”，就是自己扮演孝顺听话的好学生，把治疗师投射为自恋虚伪的父母。然后他们通过拖延完成治疗任务、当面不说治疗师背后八卦治疗师，来攻击、贬低治疗师。

6）色情移情的修通是长期任务。在治疗中期，性创伤处理的前后，不少来访者会出现冲动性行为或者强烈的色情性移情，江湖传说“美丽的文艺女边缘”是治疗师杀手，并非虚言。杀手们的猎杀对象，一般是男性治疗师，有较高自恋性，缺乏足够浪漫爱情经验和性爱经验，可能没有充分分析性爱心理发展。有一些治疗师之所以和来访者发生性关系，应该理解为一种自毁份向，是对自己职业生涯的攻击，对自己家庭的攻击，也是对学术共同体的攻击。

TFP在移情-反移情分析方面的另外一大贡献就是指出了，自体-客体关系配对会产生快速逆转，此如在治疗前半场，治疗师被投射为“抛弃性父母”，来访者扮演“空心而愤怒小孩”，后半场却变成了来访者扮演“抛弃性父母”，让治疗师认同了“空心而愤怒小孩”的角色。这一论述让我们醍翻灌顶，治疗中立即就稳定了下来，而且这也是TFP独家专卖。在这本TFPE手册中，对于“移情分析”的优劣，还结合最新文献进行了讨论。

在重男轻女背景下，女性自恋人格障碍往往呈现为害羞型自恋人格障碍，而男性往往呈现为傲慢型自恋，因为这些人格特质受到了重男轻女文化背景的强化和肯定，治疗师可能要注意两点，一是注意对这些文化进行弗洛姆所说的“社会无意识”的分析，二是治疗师往往容易出现文化反移情，比如男性治疗师容易变成文化拥护者，女性治疗师容易痛恨传统文化，无论深爱还是痛恨，都既偏移了心理治疗伦理的价值观中立位，又偏移了TFP的技术中立位。治疗师应该加强对文化无意识的自我分析和学习。

9）对某些富有人群来说，自恋、偏执、表演、反社会等人格特质，是适应其社会生活的。中产阶层出身的治疗师们往往难以共情理解，甚至把这些适应性人格特质，当作残余症状，设为治疗目标。治疗过程演变成道德改造。这涉及治疗师学习社会阶层和精神分析的关系，可以参考Class andpsychoanalysis：landscapes ofinequality（Joanna Ryan，2017）一书。
